Managing type 1 diabetes can be hard, no matter your situation. Blue Circle Health offers virtual, personalized services and support for adults living with T1D, at no cost. The program is currently enrolling adults that live in Alabama, Delaware, Florida, Maine, Missouri, Ohio & Vermont—with more states coming soon!

Blue Circle Health is a 501(c)(3) funded by the Helmsley Charitable Trust that works as an extension of your current care team for up to 6 months to help you achieve your goals and build the skills for continued success.

A few of the things to expect once you enroll:

• Personalized care plan tailored to your needs

• Support from doctors, nurses and diabetes educators

• Assistance with insurance, prescriptions, and diabetes education

• Diabetes-supportive counseling and peer support

• Social work support for housing, food, and employment issues 

Eligibility requirements:

• Diagnosis of type 1 diabetes

• 18+ years old

• Residing in one of the states listed above

• Available in English or Spanish

• Not currently pregnant

  All of Blue Circle Health’s services are completely free to participants, thanks to the generosity of the Helmsley Charitable Trust – a foundation with a mission to improve the lives of people with T1D.
